Grok predicts likely text. When it lacks grounding, it produces plausible text instead of admitting a gap. That is what people call a hallucination.
Where errors cluster
- Recent events outside the model''s knowledge or search window.
- Specific numbers, citations, dates, and quotes.
- Niche technical APIs and version-specific behaviour.
Prompt changes that help
- Ask it to say "I don''t know" explicitly when unsure.
- Paste the source text and ask for answers grounded only in it.
- Request the reasoning steps, then check the steps rather than the conclusion.
- Ask the same question in a fresh chat — long threads drift.
Check it is actually the model
Truncated or nonsense answers during an incident are not accuracy problems. Check the status page and Grok is not responding first.
